This blog is a continuation of another blog. I would strongly recommend to read the first blog to understand the exchange process of Indian DL for Polish DL.
Use Google Chrome browser so that the page can be translated from Polish to English for your convenience.
Register in PWPW following the steps HERE and once you successfully register, login to PWPW and start the online application. The application is to get the PKK number. You can also save the application as a draft and continue another time.
Scroll to the left or right side of all following images for English Translation.
Application Type
Select the Voivodeship and City Office based on your local address.
Select the below option in the drop-down menu as you are applying for a driving license. Another option is for vehicle registration:

As you are applying for exchanging Indian Driving License by attempting only Theory test, select the below option:

Section A
As I mentioned before, the data provided at the time of registering in PWPW will be used for auto-populating data in the online application. You do not need to fill any of the fields in this section. Verify the details are correct; otherwise, edit in the profile section of PWPW and return to application.
Section B
This section is to fill your Indian Driving License details.
Numer prawa jazdy: Make sure to enter the DL number as in the Original Document.
Numer druku: This is mostly a sequence number printed on the Driving License which is different from the actual DL number. Mostly appeaing on the front cover page of book type Indian DL. If you do not see any such number on your driving license, just keep this field empty or fill your DL number.
Uprawnienia na wydanym dokumencie: This option is to declare the driving license categories you posses in your Indian driving license. Select B if you have 4 wheeler license, A for 2 wheeler category. You can declare if you have both, even if you are applying only for 4 wheeler (Cat. B) in Poland.
Section C
Select the category for which you are applying in Poland.
A for Motor Bike
B for Car/4 wheelers
Please note, you have to write two Theory test separately for A and B.
Section D
Section D is the most important one where you will upload set of documents which you can prepare according to the instructions I explained in STEP2 in other blog. STEP2 is HERE 🙂 Then filling Section D will be very easy.
You can skip below two fields as there is an option to pay the application fee at a later stage.

If you are reattempting with a new application after the previous application was rejected or as per instructions from the processing officer, fill the above two fields with the previous payment details. You can also fill in details of the bank transfer or local office payment if you have already done so. I strongly recommend online payment so that all status is with the application in the first attempt itself.
After uploading the supporting documents, below is what I see in my application: A total of 2078 KB which successfully went through.

These sections will appear only if you select different categories than A or B.

If you are applying for Category A and/or B, select below declarations.
Payment
Important Note: Many people reported that below screen never appeared during the application process and the submitted application has been returned back to fill the payment details. If you do not see below screeen, DO NOT submit the application. Make the payment via info-car and upload the proof in Section D.
If you prefer to make an online payment, select Płatność Online button and follow the instructions.
If you prefer to do a bank transfer or payment in a local office or reattempting after a rejected application, click Dalej and proceed.
A total of 102.50 zl to pay inicluding the service fee of 2 zl.
Submitting the application
You need the Trusted Profile here to sign the application online.
Select the option Profil Zaufany (ePUAP) from the drop down and click the button Podpisz wniosek.
Click on the button Podpisz wniosek will redirect you to the Trusted Profile website and completing the steps there will redirect you back to PWPW.
The application status will be as below
From now, you just need to wait for the status change in the application which you can see under the Moje wnioski after login into PWPW.
Get PKK – Driver Candidate Profile Number
After successful verification of your submitted application, you will find a link appearing in the column Profil Kandydata na Kierowcę.
You can download PKK number similar to below. Just note down the number.
The Driver Candidate Profile (PKK) is generated after submitting an application for authorization or ex officio in the case of a referral for a qualification check. PKK is a unique number and is necessary to register for the exam at the Provincial Road Traffic Center (WORD). You can find more details about registering for the Theory Test at the latest stage of this article.
PKK contains the following set of information: Personal data, category of entitlements from the application, medical/psychological examinations if any required, photo, driving restrictions, entitlements required in the case of applying for the resulting entitlements (eg Cat B when applying for Cat C).
The PKK number should be presented at the Selected Driver Training Center when registering for the training, as well as at the Provincial Road Traffic Center when registering for the exam.

There is no category A as such. I understand we need to apply for category B for cars. Should we apply for A2 for 2-wheelers?
Hi Vijay, Category A for two-wheelers does exist. If you are exchanging, you may choose either of below according to your existing Indian DL category. A is suitable for most.
Category A:
Motorcycle – a single-track or multi-track motor vehicle with a side-car
Light quadricycle – a quadricycle, the unladen weight of which does not exceed 350 kg and the design limits the travel speed to 45 km / h,
Moped – a single-track or two-track vehicle equipped with an internal combustion engine with a cylinder capacity not exceeding 50 cm3, the design of which limits the speed to 45 km / h,
Category A1:
Motorcycle with an engine capacity not exceeding 125 cm3, a power not exceeding 11 kW and a power-to-weight ratio not exceeding 0.1 kW / kg.
A three-wheeled motorcycle with a power not exceeding 15 kW.
Moped – a single-track or two-track vehicle equipped with an internal combustion engine with a cylinder capacity not exceeding 50 cm3, the design of which limits the speed to 45 km / h,
Light quadricycle – a quadricycle, the unladen weight of which does not exceed 350 kg and the design limits the travel speed to 45 km / h,
Category A2:
Motorcycle – with a power not exceeding 35 kW and a power-to-weight ratio not exceeding 0.2 kW / kg, but it cannot be created as a result of introducing changes to a vehicle with a power exceeding twice the power of this motorcycle,
A three-wheeled motorcycle with a power not exceeding 15 kW,
Light quadricycle – a quadricycle, the unladen weight of which does not exceed 350 kg and the design limits the travel speed to 45 km / h,
Moped – a single-track or two-track vehicle equipped with an internal combustion engine with a cylinder capacity not exceeding 50 cm3, the design of which limits the speed to 45 km / h,
Category AM:
Moped – a single-track or two-track vehicle equipped with an internal combustion engine with a cylinder capacity not exceeding 50 cm³, the design of which limits the travel speed to 45 km / h.
Light quadricycle – a quadricycle, the weight of which does not exceed 350 kg, and the design limits the travel speed to 45 km / h.

Hi, to obtain pkk people are suggesting to go offline where we need medical certificate(vision, hearing & balance test). I see in above steps while filling it online you have not mentioned about any medical certificate. I am little confuse here.

Will I get the update from Urzad to do the payment and resubmit the application or should I submit the application from fresh again?
Hello Viren, Thanks for your comment. Missing payment page is either a bug in the pwpw system or due to weekend maintenance. You have two options, wait for processing officer to return the application to update payment OR you submit a new application this time by attaching payment proof.

Hello Sreenath, I have paid the money via millennium online and have attached the confirmation on the attachments.
The payment column shows lack, will the officer check it and would be fine or is info car the only option to do.

THanks, Sreenath, your blog was really helpful in making the application. Want to share my experience that might be an update to the process. The online payment via infocar was complicated in Warsaw. In Infocar, only 4 offices are listed in Warsaw and these are outside. I did not submit application as per the instruction you gave, and visited the office to enquire (and probably generate PKK in person). I was told that payment is not needed for PKK number (and without an interpreter the office was not taking application). So i returned and submitted online without making payment. In 2 days, i got my PKK number and a separate email from office giving details on how i should make payment of 100,5 PLN after i pass the test and before requesting printed license. This info might be useful for Indians applying in Warsaw.
Hello Yuvaraj, Thanks for sharing your experience. The online payment form is intermittently missing from the application navigation. If this online payment form is available, then there is no need to go to infocar website. I also mentioned, the payment can be directly to your selected city office bank account as normal bank transfer; but for that you first should know the correct bank account number. Yes, I have feedbacks from many that the payment is not sometimes required to get PKK. But please note, this is not exempting anyone from paying later 🙂 I hope your feedback will be helpful for many. Zameldowania is mandatory as far as I know. Please note, cities are having different requirements for time to time. I would suggest you take Zameldowania.

Hi, just to update other readers, I had to go to Urząd Miasta, Krakow to collect my driving license. I checked the status of the license on their website and it said something like ready to be collected. I just went and collected mine then from the main transport office. I had followed the process and submitted the application.
But I received the comment that NO SCAN OF A DOCUMENT CONFIRMING THE RIGHT OF RESIDENCE IN POLAND FOR AT LEAST 185 DAYS.
I had given my visa and zameldowanie as part of the supporting document.
It has not been 185 days since my arrival into Poland, but I am told that after 185 days I will not be able to drive.
My question is – when can I request the PKK number? and will I be able to drive in Poland after 185 days of arrival into Poland?
Hi Punit, The decision is with Processing Officer and the city department. Some offices accept Visa some don’t access Visa and they insist for Karta Pobytu. As the message is very clear, you may submit the application once your 185 days stay complete in Poland.

Hello Ashwini, I am not sure if there is expiry date for PKK as such. May be infocar was introduced later as I understand your PKK is from 2019 year. Worth checking directly with Transport Department at city hall.

Hi Chirag, Please read the blog. I explained there. Some district offices are still not integrated to the online payment service provider and could be the reason the page did not appear. Nothing to worry, the processing officer will contact you to make bank transfer or payment at the time of colelction of DL..
Hi, I have a complicated situation. I initially applied for PKK with an intension applying from scratch for driver’s license in Poland. I passed the theoretical test. However, the cost of practical + translator is too expensive. So, since I already have driving license from my home country, I would prefer to exchange it rather than take practical. How to apply for it in this case?

Hello Avishek, Some of the transport office departments in certain cities are not affliated to PWPW or Infocar; in that case you may need to transfer the fee to bank acocunt of the specific city office and then atatch the proof in the online application. For some of the applicants, the PWPW website allowed to submit the application and they later paid during the DL collection.

So I followed your blog and got my PKK number for both categories B and A. I requested for only B but anyhow got both I think based on my Indian DL.
Now I have cleared cat B theory test last week, so now do I need to clear the cat A as well or is it fine if I skip it?
