If you live and work in Poland, you are likely paying into the National Health Fund (NFZ). But did you know those benefits follow you across the border? Whether you are heading for a ski trip in the Alps or a summer holiday in Spain, you need the EKUZ (Europejska Karta Ubezpieczenia Zdrowotnego).
In this guide, I’ll show you how to get it for free, how to cover your family, and a secret tip for long-term residents to get a card that lasts 5 years.
What is the EKUZ (EHIC)?
The EKUZ is a free card that proves you are insured in Poland. If you have an accident or sudden illness while visiting an EU/EFTA country or the UK, it allows you to use the local state healthcare system on the same terms as a local citizen.
Who is Eligible?
- Anyone employed in Poland or running a business (paying ZUS).
- Spouses and children of insured persons (dependents).
- Pensioners and students.
Step 1: Ensure Your Family is Registered (The “ZCNA” Step)
Before applying, especially for a spouse or child, you must ensure they are officially “linked” to your insurance.
- The Rule: If your spouse doesn’t work, they are not automatically insured. You must ask your HR department to submit the ZUS ZCNA form.
- How to check: Log into PUE ZUS (Ubezpieczony tab -> Dane członka rodziny) or check the IKP (Internetowe Konto Pacjenta) of the family member. If the status says “Active/Ubezpieczony,” you are ready.
Step 2: How to Apply (3 Easy Ways)
1. The Instant Way (In-Person)
Need to travel tomorrow? Go to your local NFZ Oddział Wojewódzki.
- Process: Bring your Karta Pobytu or ID.
- Result: They usually print and hand you the card on the spot.
2. The Lazy Way (Online via IKP) – Recommended
- Log into pacjent.gov.pl (using Profil Zaufany).
- Go to “Moje konto” -> “Ubezpieczenie zdrowotne za granicą.”
- Fill in your mailing address, and the card will arrive in your mailbox in about 5–10 days.
3. The Mobile Way (mojeIKP App)
- You can now apply directly through the mojeIKP app on your smartphone. It’s just a few taps!
Pro-Tip: How to Get a 5-Year Card ⏳
Usually, EKUZ cards for workers are issued for 3 years, and for spouses for 18 months. However, if you have a Long-term EU Residence Permit (Karta Rezydenta Długoterminowego UE), you can get a longer validity.
The Trick: When applying, attach a short cover letter (in Polish) stating that you hold a long-term resident status and request the card validity to match your status.
Example: “Proszę o wydanie karty na 5 lat ze względu na posiadanie statusu rezydenta długoterminowego UE.”
Special Rules for Kids & Students
- Children under 18: Great news! Their EKUZ is usually valid until their 18th birthday. You won’t need to renew it every year.
- Students (18–26): If your child is still studying, they can stay on your insurance. You must attach a scan of their Student ID (Legitymacja) to the application. These are usually issued for 12 months.
Summary Checklist for Families
| Person | Validity | Key Requirement |
| Worker / Employee | 3 Years | Active ZUS contributions. |
| Long-term Resident | 5 Years | Attach request letter + Resident Card copy. |
| Non-working Spouse | 18 Months | Must be registered via ZUS ZCNA form. |
| Children (<18) | Until age 18 | Must be registered via ZUS ZCNA. |
| Students (18-26) | 12 Months | Provide valid Student ID. |
Final Reminder: EKUZ is for state emergencies. It does not cover private clinics or mountain rescue (like helicopters). For 100% peace of mind, always pair your EKUZ with a small private travel insurance policy!
